In accordance with Texas Health and Safety Code § 181.105, the following information is provided regarding your health care records and your rights as a consumer.

REQUESTING YOUR HEALTH CARE RECORDS

You may request a copy of your health care records from Moments of Growth Counseling PLLC.

To request your health care records, please send a written request by email to:

records@momentsofgrowthcounseling.com

Please include your name and a description of the records you are requesting. For your privacy, please do not include sensitive health or clinical information in your initial email.

After your request is received, you will be contacted regarding any applicable copying fee and the delivery of your records.
